CST CLINICIAN RECOGNITION AWARD - NOMINATION FORM

This award is designed to recognize exemplary commitment to the CST mission through excellence in front-line clinical innovation and program development, humanitarianism, and/or education as part of the direct case of Canadian organ transplant patients.

ELIGIBILITY CRITERIA

Nominee must be a CST Member (Full or Associate) in good standing for the past 5 years
Nominee must be a Canadian Resident
Academic Rank is not required
Previous Winners Excluded: Yes, previous lifetime achievement, research excellence and education and teaching excellence award winners excluded

LIST OF SUPPORTING DOCUMENTS required to complete submission to be sent to CST Office by email in a single PDF to: admin@cst-transplant.ca and received no later than March 31, 2025:
